If you are being evaluated for an insurance covered procedure please bring your insurance card to your visit. Check with your insurance carrier for any applicable co-payment and deductible required. Co-payment and deductible are due at the time of the appointment.
For your convenience, we accept Cash, Visa, MasterCard, Discover, and American Express.
Cancellations & Missed Appointments
If you are unable to keep an appointment, we ask that you kindly provide us with at least 1 business day notice. This courtesy on your part will make it possible to give your appointment to another patient.
Appointments not kept or canceled on only a few hours notice typically prohibit offering the session time to another person. There is no charge for missed appointments, however there will be a $25 cash payment required for a rescheduled appointment.
